CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are looking for a Band 7 Complex Case Manager to join the St Albans and Harpenden Complex Case Management Team on a fixed-term contract until March 2028.
This is a part-time role (22.5 hours per week), offering the opportunity to work within a dedicated and supportive team.
You will job share with another Band 7 Complex Case Manager and work alongside a Band 4 Complex Case Co-ordinator.
This is a varied and rewarding position where you will:
- Deliver high-quality case management activities related to assessment and treatment, identifying gaps in the care provision, planning and evaluation of patient care
- Support individuals with long-term conditions and complex needs
- Contribute to the development of clinical guidelines and service improvements
- Operate in line with Trust policies and professional standards
Flexibility is required in this role, and there may be occasions where you will support teams across different bases or localities.
- You will be either a Registered Nurse (RN) or Allied Health Professional (AHP) with NMC Registration or HCPC Registration
As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
