**NOTE: We are unable to accept applications from newly qualified nurses who have not completed a 12 month preceptorship since qualifying**

To act as named practitioner/care co-ordinator for a specific group of patients and provide care under the supervision of the Care Home Liaison/ Dementia Primary Care Team Lead. To deliver Assessment and Treatment post diagnosis of dementia within the community to this group of patients and liaise with other professionals regarding progress and outcomes.

To act as practitioner/care co-ordinator with responsibility for the assessment, planning and delivery of care to the identified patient group.
