Nottinghamshire’s Education, Health and Care (EHC) Assessment Service is a county wide service with responsibility for carrying out the statutory functions for Education Health Care Needs Assessments (EHCNA) and maintaining all Education, Health and Care Plans (EHCPs) for children and young people with Special Educational Needs and Disabilities (SEND).
We are currently looking to recruit several district-based EHC Practitioners to the team. You will be a highly motivated practitioner with the ability to work under pressure. As a team player, you will need to demonstrate your enthusiasm and person-centred approach to secure the best possible outcomes for children and young people with SEND.
As an EHC Practitioner you will support children and young people with SEND by managing the statutory assessment process and ensuring the timely development and review of EHCPs. The role involves working in collaboration with families, education settings and multi-agency professionals to ensure that the support and provision is tailored to meet individual needs and statutory requirements.

You will be required to: Have a strong working knowledge of the Children & Families Act 2014, the Special educational Needs and Disability Regulations 2014, the SEN Code 2015 and the Care Act.
- Construct and develop education, health, and care plans based on approved assessments. Ensuring that CYP with an EHCP have suitable provision in place with appropriate monitoring and accountability.
- Build and maintain strong relationships with children, young people and families using a strength-based approach. Provide guidance and advice to service users and carers regarding EHC plans.
- Clearly communicate decisions to families and professionals, ensuring reasons are evidence-based and outcomes are explained in accessible ways.
- Coordinate annual reviews and monitor service user progress, including transitional reviews when a child or young person is transferring from one stage of education to another.
- Identify required support services and request funding where appropriate.
- Ensure all records are maintained in compliance with council policies and statutory requirements including clear and up-to-date documentation of case progress and decisions.
- Work collaboratively with health, education, and social care professionals to ensure comprehensive service delivery.
- Experience in problem-solving, mediation, and conflict resolution.
- Provide oversight and guidance to EHC Assistant Practitioners, supporting quality assurance and workforce development.
