We are seeking to appoint a number of GPs with an interest in frailty to support the delivery of various innovative Frailty Services as part of the North Yorkshire Community Frailty Hub.
Applicants must hold GMC registration and have experience in triage and telephone assessment. Experience working in Geriatric Medicine is desirable. Successful candidates will join our growing team of GPs with an Extended Role (GPwER) in Frailty and will be supported to work towards the Diploma in Geriatric Medicine, alongside participation in our regular frailty education sessions.
The population aged over 85 in York is projected to increase by 53% by 2033, and we are preparing to meet the needs of a growing number of patients living with frailty. The Community Frailty Hub is a collaborative multidisciplinary team of health, social care and voluntary sector partners supporting patients across York and North Yorkshire during times of crisis, through discharge support, and through proactive MDT caseload management, including the delivery of Comprehensive Geriatric Assessments (CGA).
We work closely with our external partners to deliver care through a no wrong door approach, ensuring patients receive coordinated, integrated and holistic support.
This is primarily a face-to-face role based at Gateway 1, Holgate, York, working between 8:00am and 5:00pm. Clinicians will use SystmOne as the clinical system.
