We are looking for a motivated health care support worker to join our 20 bedded intermediate care rehabilitation ward, supporting patients with frailty and complex needs during their rehabilitation journey.

Our ward provides holistic, multi-disciplinary care focused on rehabilitation and discharge to assess patient providing for their well-being whilst in our care. Main duties of the job
It is expected that the post holder will be able to carry out all of the duties listed in the main duty section below, but will not necessarily do so. The duties will vary from ward to ward. They may carry out other similar duties in support of the ward environment which are not listed. No duties are to be undertaken by the post holder unless they have received the appropriate training or instruction.
Deliver sensitive, appropriate and high quality patient care under the direction of qualified nursing staff which respects the patient’s privacy, dignity and religious and cultural beliefs.
Maintain and promote a safe working environment for patients, visitors and staff, with particular reference to infection control and health and safety.
Contribute to the effective management of information ensuring it is handled and exchanged with care, accuracy and sensitivity, maintaining patient and clinical confidentiality at all times.
Assist in maintaining stores at agreed levels and ensure that repairs and replacements are ordered by appropriate reporting.
Maintain and promote effective communication both within the multidisciplinary care team and with other colleagues, patients, relatives and visitors.
Provide personal care to patients.
Monitor observations including heart rate and blood pressure.
Calculate and escalate observations as per NEWS scoring system to appropriate staff.
Support patients with regaining mobility and ability as per therapy goals.
Wound observation and monitoring of skin integrity.
Wound dressings and care.
Support with nutrition - with knowledge of modified diets and fluid.
Monitor fluid input and output for patients and escalate as appropriate.
Continence care. Catheter care.
Collect urine samples- MSUs and CSUs and perform urinalysis.
Phlebotomy.
Perform and record blood glucose monitoring.
Stoma care.
Microbiology swabbing.
Perform ECGs.
Assist to provide emotional/ psychological support to patients and their families.
Support and train bank/ agency staff.
Maintain and order supplies of clinical equipment.
Carry out monitoring and auditing (in relation to infection prevention, NEWS scoring).
Provide support to trained staff and complete assessments in care plans under RN supervision – falls assessments, bed rail assessments and incontinence assessments.
Assist with the admission and discharge of patients to/ from the ward.
