Symphony Healthcare Services is recruiting a Health Care Assistant (HCA) to our existing team within the West Somerset Primary Care Network, based at Williton Hospital and Minehead Medical Centre.
This an exciting opportunity for a trained HCA interested in improving patient outcomes by working within our dynamic community based Living Better Neighbourhood project (based at Williton Hospital), being an advocate for cross organisational working.
Within the GP surgeries, you will be responsible for performing NHS Health Checks, you will play a vital role in delivering health checks and cardiovascular assessments to patients, helping to identify those at risk of developing serious health conditions like heart disease and diabetes.
You will also perform phlebotomy and other HCA duties as detailed in the Job Description; both within surgeries and with the PCN central team, working with patients in the community and care homes.
The successful applicant will have excellent communication skills, be a good team player, and be able to assist colleagues to ensure the highest possible standard of care is provided to our registered patients.
The post available is a for a fixed term contract for 24 to 37.5 hours per week, depending on the candidates preference.
The rate of pay for this role is £12.71 to £13.34 per hour. Starting salary will be offered based on previous experience.
As an HCA in a primary care setting, you will play a vital role in delivering high-quality patient care and supporting the clinical team in the day-to-day running of the practice.
Working under the guidance of the nursing team and GPs, you will carry out a range of clinical tasks such as phlebotomy, blood pressure monitoring, ECGs, health checks, minor dressings, and assisting with chronic disease management.
You will also contribute to the overall patient experience by providing reassurance, health promotion advice, and practical support throughout their visit.
This role is ideal for someone with previous HCA experience, preferably in primary care or community health, who is enthusiastic, organised, and committed to making a difference in the lives of patients.
Good IT skills are required, and knowledge of EMIS Web would be desirable. The post holder must also be willing to undertake any training required to further develop their skills and competencies
The role bridges patient care and operational support, focusing on patient comfort, health promotion, and maintaining clinical environments.
