We're seeking a dedicated Hospital Liaison Healthcare Assistant to join our compassionate and diverse team in Kings Lynn team- a group of professionals committed to supporting individuals in at the Queens Elizabeth Hospitasl Kings Lynn with compassion.
The post holder will work across the hospital site (e.g. A&E and admissions wards) to provide rapid access to assessment, brief interventions/harm minimisation and support discharge planning/onward referrals for people attending hospital with substance misuse related issues.

Hours full time, 37.5 hours per week
Full Time Salary £25,077-£26,502 dependent on experience (based on full time working hours, pro rata if part time)
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

About the role
- Working closely with your Nursing team to develop pathways to care
- Support service users in opiate and alcohol interventions within a hospital setting.
- You will support on harm reduction interventions along with physical and mental health assessments.
- Keeping the people who use our services at the heart of everything you do
- Good interpersonal skills and a willingness to work flexibly as part of a team
- Experience of working within substance misuse or a social care setting would be an advantage but not essential
- A clear understanding of the need for and ability to deliver quality services
- Good organisational skills
- Confidence to work on own initiative as well as part of the team
- 25 days holiday (+ bank holidays) rising by 1 day for each years’ service “Capped at 30 days”
