An exciting an opportunity has arisen to join the LGT home visiting service as the Home Visiting and Wound Care Lead.
The post holder will work closely with the Head of Service, Deputy Head of Service, wider Foot Health Leadership Team and Wound Care Leads to deliver this core branch of our service.
The service offers its team regular clinical supervision, team meetings and protected learning time. In addition, we are happy to support the individual learning needs of the post holder. We also provide monthly leadership meetings and training for increased support,
To operationally manage the foot health home visiting service in Lewisham and Greenwich NHS Trust. Including evaluation of all domiciliary referrals, triaging all referrals to ensure only housebound patients are reviewed at home, based on podiatric need and medical risk.
As a key member of the Multidisciplinary Wound care team, you will be a lead clinician in the management of a designated wound care caseload
in the community. Taking on full responsibility and duty of care for the most complex and challenging patients in an area specific caseload, you will also use your expert clinical knowledge and skills to provide direction and support to other team members.
Working closely with the principal podiatrist for wound care, to lead and provide support and advice to colleagues on house bound patients
including those on the end of life pathway.
To line manage Band 5/6 podiatrists, undertaking their appraisals and clinical supervision, encouraging professional development,
ensuring team members are up to date regarding mandatory training.
To deputise for the Head of Podiatry, or fellow service leads, when required.
To work as an integral member of the Integrated Foot Health wound care team working in the Acute Foot Service at University Hospital Lewisham.
To support the triage of the wider Foot Health Service caseload.
