This role is key to ensuring that quality standards are achieved, and you will support individuals on a one-to-one or group basis using person-centred approaches to meet personal outcomes. You will respond to crisis and emergency situations appropriately, ensuring confidentiality of information according to Council County policy & relevant legislation. You will also work with individuals, carers, specialist workers and relevant agencies to contribute towards the completion, review and ongoing monitoring of care plans, based on effective consultation.

Hub Enablement Workers are key people working in a supportive team environment – you will become a valuable member of the day opportunity team and be supported to undertake high quality training and are expected to adhere to confidentiality, engage in and attend staff meetings to contribute towards the performance of Day Opportunities.
