We are seeking an experienced and motivated INT Transformation Manager to lead the development, delivery and evaluation of Integrated Neighbourhood Teams (INTs) across Nottingham City.
Working closely with clinical, operational, community and system partners, you will support the roll-out of this major transformation programme, building on the Pathfinder Blueprint developed in Nottingham City East and Bestwood & Sherwood.
This is a pivotal role focused on shaping service redesign around local needs and priorities while ensuring strong programme governance, impact measurement and clear reporting of outcomes.
You will provide senior transformation, project and programme management expertise to drive integrated working, service improvement and sustainable change across CityCare and the wider Nottingham City system.
