Job Purpose
The post holder will support the delivery of medicines optimisation across community healthcare services. Working as part of the Medicines Management team, the role ensures the safe, effective, and cost-efficient use of medicines.
The Medicines Optimisation Pharmacy Technician will:
Provide advice and support to colleagues on medicines within professional competence.
Maintain accurate records and manage medicines-related documentation in line with governance, legislation and organisational policies.
Monitor and review data, identify trends, and support the organisation to improve practice, ensure compliance, and enhance patient safety.
Contribute to staff training, development, and continuous quality improvement initiatives related to medicines optimisation.
Work independently and as part of the team to manage the teams workload and priorities
Dimensions
Provides support to medicines optimisation across community healthcare services, contributing to the safe and cost-effective use of medicines.

Maintains accurate records in line with organisational policies, procedures, and relevant legislation.

Contributes to patient safety and quality improvement through supporting medicines management and departmental activities.
Key Responsibilities
Governance, Safety & Compliance
Support compliance with medicines legislation, policies, and procedures to ensure safe and effective practice.
Maintain accurate records, audit trails, and documentation to meet governance and regulatory requirements.
Support the identification, investigation, and resolution of medicines-related incidents, risks, and safety alerts.
Contribute to the development and review of medicines-related policies and procedures.
Medicines Optimisation
Support the safe, effective, and cost-efficient use of medicines across services.
Maintain medicines-related systems and records, including prescribing governance processes.
Work with stakeholders to promote best practice in medicines use.
Data, Audit & Quality Improvement
Assist in monitoring data and supporting improvements in practice and compliance.
Collect and analyse data to identify trends, risks, and opportunities for improvement.
Contribute to quality improvement initiatives to enhance patient safety and service delivery.
Administration & Service Support
Maintain accurate records in line with policies, procedures and legislative requirements.
Provide administrative support, including coordinating meetings, preparing documentation, and tracking actions.
Organise and prioritise workload to support delivery of team objectives and work plans.
Communication & Team Working
Communicate effectively with colleagues and stakeholders within scope of role.
Respond to medicines-related queries within their own competence, seeking guidance where appropriate.
Work collaboratively across teams to support medicines management and service delivery.
Training & Development
Support delivery of training and provide guidance on medicines-related processes within scope of role.
Contribute to the development of staff and promote a culture of continuous learning.
Professional Practice
Work independently within professional standards, policies, and scope of practice.
