Description
Surrey and Borders is looking for someone who is passionate about ensuring a high-quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives.

Working Location:

Working Pattern: Monday to Friday from 9AM to 5 PM

  • Lead the clinical management of care packages for individuals using the service.
  • Coordinate and monitor person-centred healthcare for a designated group of service users.
  • Deliver psychosocial interventions through individual and group therapy.
  • Work as part of a multidisciplinary team (MDT) to assess needs, plan care, and respond to changes in service users’ health.
  • Provide therapeutic activities, emotional support, and stability to promote engagement and recovery.
  • Build and maintain professional relationships with staff, external professionals, and agencies.
  • Respond effectively to unpredictable situations, including crisis incidents and challenging behaviour.
  • Maintain accurate documentation and communication with service users, carers, relatives, and professionals.
  • Support quality improvement, service development, and adherence to organisational standards and policies.
  • Conduct risk assessments and contribute to risk management and safeguarding processes.
  • Demonstrate knowledge and application of relevant legislation, including the Mental Health Act (MHA), Mental Capacity Act (MCA), and Deprivation of Liberty Safeguards (DoLS).
  • Support staff development, student mentorship, and team functioning.
  • Participate in management activities such as staff supervision, appraisal support, and safe staffing.
