Description
We are seeking a motivated and experienced Registered Nurse or Paramedic to join our dynamic Rapid Response Team, delivering urgent community-based care to patients in their own homes.
This role focuses on preventing hospital admissions and out of hours support, by providing high-quality clinical assessment, treatment, and coordinated care to patients experiencing acute illness or requiring short-term rehabilitation. Working as part of a multidisciplinary team, you will collaborate closely with GPs, therapists, social care, and acute services to deliver seamless, patient-centred care.
As an autonomous practitioner, you will undertake comprehensive assessments, initiate care plans, deliver clinical interventions, and coordinate complex care packages. You will also contribute to the day-to-day leadership of the team, supporting operational delivery and mentoring junior staff.
This is a fast-paced, rewarding role requiring strong clinical judgement, excellent communication skills, and the ability to respond rapidly to changing patient needs. The service operates 24/7, 365 days a year, so flexibility to work shifts, including nights and weekends, is essential.
NELFT is a supportive and inclusive organisation built on its core values of being kind, respectful, and working together with communities, ensuring a positive culture for both staff and patients.
Staff benefit from working in a Trust that promotes continuous professional development, strong teamwork, and high-quality patient care across integrated services.
As part of the NHS, employees receive a competitive salary with High Cost Area Supplement (HCAS) where applicable, generous annual leave, and access to pension and wellbeing support. New starters will typically be subject to a probationary period, providing structured support and development to help them succeed in their role.
