Nottinghamshire County Council have developed a new brokerage function and are now ready to recruit a long-term brokerage team to support the people of Nottinghamshire find a quality care for life.
Are you passionate about making a difference in adult social care? We’re looking for dedicated and detail oriented Care and Support Brokers to join our team.
In this vital role, you’ll work closely with social workers, care providers, and families to source and secure appropriate care and support to meet individuals’ assessed needs. Your work will directly support timely hospital discharges, promote independence, and ensure high-quality care outcomes at best value for Nottinghamshire County Council and the people we support.
Key Responsibilities
- Coordinate and broker care and support in line with care plans and funding agreements.
- Liaise with care providers to negotiate suitable care and support, costs and ensure availability.
- Maintain accurate records and ensure compliance with procurement and contracting policies.
- Support the delivery of cost-effective care solutions that meet personal needs and promote independence.
What We’re Looking For
- Experience in adult social care, brokerage, or commissioning.
- Proficient communication and negotiation skills.
- Ability to manage demanding priorities in a fast-paced environment.
- Knowledge of care market dynamics and relevant legislation is desirable.
