Riverside Surgery is situated at Port Talbot Resource centre and is one of 4 GP surgeries housed in the building. Our practice serves a diverse patient population of over 9, 500 patients.
Our growing multi-disciplinary team, which includes Doctors, Advanced Nurse Practitioner, highly skilled nurses, prescribing team as part of the care navigation and administration team who are committed to delivering high-quality, patient-centred health care while embracing innovation and collaborative working. We also undertake vocational training of new practice nurses as part of the Wales Academy for learning in Primary Care and Swansea University.
Hold clinics for patients requiring face-to face or telephone Structured Medication Reviews (SMRs), prescribing, monitoring, and optimising medicines to manage long-term conditions.
Contribute pharmaceutical advice for the development and implementation of new services/systems that have medicines/prescribing components (e.g. Chronic Disease Management, QI projects, LES).
Provide management, support and training for prescription administrators.
Contribute to public health campaigns, including flu vaccinations, and other adult immunisation programmes.
Undertake changes to medicines (switches) designed to save on medicine costs in accordance with local formularies, or in response to disruptions in the medicines supply chain.
Answer medicine-related enquiries or requests from GPs, other practice staff, and patients.
Analyse, interpret and present medicines data to highlight issues and risks to support decision making.
Undertake audits of prescribing, feedback the results and implement changes in conjunction with the practice team.
Monitor practice prescribing against the local health board formulary and make recommendations to GPs for medicines that should be prescribed by hospitals only or subject to shared care arrangements.
Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in care homes).
Undertake any other duties appropriate to the grade.
Participate and support personal development reviews and objective setting with your clinical line manager.
Maintain registration and revalidation with the General Pharmaceutical Council by regular updating, self-directed learning, and attendance at courses that would help in personal, and service, development.

We are based on one site in the purpose- built Port Talbot Resource Centre which also houses several other services under its roof.
We offer a range of NHS services in the Practice via our multidisciplinary and highly skilled team. We currently have 2 GP partners, 5 salaried GPs, an advanced Nurse Practitioner working in the Practice Manager role, a cluster pharmacist, two practice nurses, cluster Mental Health Workers, social prescribers and physiotherapists. We also employ a highly skilled and very experienced and dynamic care navigation team which is part of the prescribing team.
