Description

We are looking for an enthusiastic, experienced and dedicated Practice Nurse to join our friendly, forward thinking primary care team. This is an excellent opportunity for a motivated nurse who is passionate about delivering high quality patient care and contributing to the ongoing development of our clinical services. The successful candidate will play a key role in providing a wide range of nursing services within the practice, including routine clinics and chronic disease management.

Essential

Registered Nurse - NMC
Experience in primary care or a strong interest in developing skills in this area.
Competence in cervical screening, immunisations, and wound care or willingness to undertake training
Strong communication, organisational, and clinical decision making skills.
Commitment to ongoing professional development.
Driving licence - for home visits and to be able to work across both our sites- Woolston Lodge Surgery and Chartwell Green Surgery.
Desirable Skills

Desirable

Experience in chronic disease management clinics.
Knowledge of SystmOne
Knowledge of Quality Framework Outcomes (QOF)
