Description: Full Time / 37 Hours Per Week / Permanent The Reablement Service are a small but dedicated team, whose main goal is to enrich the lives of those returning home from hospital or experiencing a short-term change in needs at home, to ensure they maintain their independence and healthy living in a safe manner within the comfort of their own home. We are looking for outstanding individuals which can match the services drive and ambition as one which has been rated outstanding by CQC. In addition, we're responsible and accountable for the planning and programming of calls for the Reablement Provider Service by using the Electronic Rostering and Monitoring system. Thus, providing timely performance data in terms of outcomes, unit cost and the quality-of-service provision. As a Reablement Co-ordinator you'll join a team of 80 plus, including: Front line staff, Home First Assistants, Home First Officers, Reablement Co-Ordinators and administrative employees. There is full time, permanent and fixed term roles available working 37 hours per week. You'll be based at the Neville Centre on Leicester General Hospital site. Working a mixture of morning 7.30am to 4.00pm and evening shifts 1.45pm to 10.15pm across a rota covering 7 days a week, this includes weekends, bank holidays and other public holidays. What you'll be doing As a Coordinator, you'll be instrumental in the planning of all support visits within the Reablement Service across the Leicester area to allow people to remain as independent within their own homes as possible, to avoid hospital admissions, reduce ongoing support and to live with respect and dignity. Day-to-day activities will start with shift handover to colleagues, ensuring you've clarified all outstanding visits with residents that are still outstanding awaiting cover. Following this, you'll prepare to take phone calls from staff and public. In addition, you'll will keep track of requests and actions that come through via email. Prioritisation is key within this role as all requests should be responded to within 2 hours. Following this you'll then be required to enter care plans on the OneTouch our current Electronic Rostering System and allocate the visits. You'll be in contact with co-workers and front-line staff regarding the visits they are completing which will need to be recorded in a timely and accurate manner. You'll be held accountable for sending reports to alloca
ted case responsible staff, depicting the actions taken such as, cancellations due to hospitalisation or other reasons of visits. In addition, you'll receive all incoming referrals into the Reablement Provider Service. Working as part of a multi-disciplinary team, you are working to deliver Outstanding support and services to colleagues and person of Leicester. What you'll need As a Reablement Co-ordinator you'll need to be disciplined with yourself, have excellent organisation and IT skills. You must be able prioritise your workload and be able to work under pressure in a busy and often difficult job role. In addition, you'll have great interpersonal skills. You'll be able to communicate effectively to a wide range of people from varied and diverse backgrounds, ethnicities, and educations in both verbal and written contexts. As a confident person you'll be able to match your approach to the needs of each individual as they are encountered with a flexible and no-nonsense approach. Previous coordination experience within a social care setting is desirable.
