Are you passionate about supporting individuals to achieve their maximum potential and regain independence? West Berkshire Council's Adult Social Care Team is seeking a dedicated and motivated Reablement Practitioner to join our dynamic team.

The Role

Responsibilities

As a Reablement Practitioner, you will play a crucial role in delivering high-quality reablement services to our residents. Working under the guidance of the Assistant Team Manager, you will be responsible for:

  • Providing hands-on support and guidance to enable individuals to regain daily living skills, which will include personal care.
  • Collaborating with other professionals, such as occupational therapists and physiotherapists, to ensure comprehensive care.
  • Monitoring progress and providing regular feedback on individuals' achievements.
  • Delivering interventions that promote independence, self-care, and regaining skills.

The ideal candidate will possess the following skills and attributes:

  • A genuine passion for supporting individuals to achieve independence and improve their quality of life.
  • Strong communication and interpersonal skills, with the ability to work collaboratively within a multidisciplinary team.
  • Effective organisational and time management skills, with the ability to prioritise tasks and manage a diverse caseload.
  • The ability to travel across the district as required to provide care to those accessing reablement services.
  • Commitment to personal development and training.

  • The Current Hours Available Are

    Week 1: Monday 7am-10.30am, 12pm-1.30pm & 6-10pm, Tuesday 7am-10.30am, 11am-1.30pm & 6-10pm, Wednesday off, Thursday off, Friday 7am-10.30am, 12pm-1.30pm & 6-10pm, Saturday 7am-10.30am, 12pm-1.30pm & 6-10pm, Sunday 7am-10.30am, 12pm-1.30pm & 6-10pm.

    Week 2: Monday off, Tuesday off, Wednesday 7am-10.30am, 11am-1.30pm & 6-10pm, Thursday 7am-10.30am, 12pm-1.30pm & 6-10pm, Friday off, Saturday off, Sunday off.

    We would consider some flexibility in the hours applied and would welcome applications from those only interested in the morning and lunch or evening hours only.

    As part of this exciting opportunity, you’ll have the chance to undertake an 18-month Level 2 apprenticeship in in health and Social Care, to further develop your skills.

    This role requires the incumbent to have a car available at work to undertake his/her duties as they will be expected to undertake at least 1500 business miles per year. Essential Car User status attracts a lump sum allowance paid on a monthly basis as well as a free car parking space if he/she works at one of the Council’s corporate buildings.
