Description
We are seeking a dedicated Band 3 Health Care Assistant to join our Discharge to Assess and Reablement team in Mid Nottinghamshire. You will support patients returning home from hospital by delivering person‑centred care, rehabilitation, and reablement activities that help them regain independence and prevent unnecessary readmission. Working closely with nurses, therapists, and wider multidisciplinary colleagues, you will contribute to safe, timely discharge and ongoing community support.

This is a mobile, community‑based role requiring regular travel across a large county area to visit patients in their homes. You will use electronic patient records, communicate clearly and sensitively with patients and families, and respond flexibly to changing needs within a fast‑paced service. This role is ideal for someone who is motivated, compassionate, and committed to supporting people to remain safely and independently at home.

Under the direction of a registered professional participate in the development and maintenance of high-quality care through involvement in assessment, planning, implementation and evaluation.

Working at times without direct supervision in patients’ own homes, residential homes, or nursing homes.

To provide rehabilitation and care with the aim of improving function for people through individual treatment and care plans and education.

To collect relevant data that will support the effectiveness of clinical intervention.
r>To facilitate and support timely discharge and inappropriate admissions by supporting the patients to remain as independent as possible and maintain or improve their level of function and quality of life.
