Job Overview

Would you like a role working shifts Monday to Friday 8am to 7pm?

We are looking to recruit a part time Registered Nurse to join our dynamic team on Gate 24 - Plastic Surgery Dressings Clinic and Outpatients, part of the largest Division at North Bristol Trust.

The department is a small team of experienced nurses who work alongside Health Care assistants and Assistant Practitioners as well as a wider multi-disciplinary team. The department not only runs Outpatient clinics for the Plastic Surgery team but also daily nurse-led dressings clinics to support the timely discharge and ongoing wound management of post op patients from the Plastic surgery ward, 33a.

  • You will gain experience of working in a fast-paced Outpatient Department.
  • You will be supported by the wider Nursing team to provide an excellent nurse led dressing clinic service for NBT Plastic Surgery patients, seeing an extensive range of simple and more complex wounds, using a variety of dressings.
  • You will work with the wider team to jointly manage change to provide an excellent Outpatient service and patient experience.
  • You will be closely working with the Plastic Surgeons, Nurse Specialists, Clinical Psychology, Physiotherapists (Trauma, Hand and Sarcoma), Burns team, Plastics Trauma, Gate 33a (Plastics and Burns), and Dermatology to provide high quality care for patients.

  • Main duties of the job

    • Working within a small team in an Outpatients setting
    • Running Plastic Surgery Outpatient Clinic with Doctors, Nurses and AHP's
    • Running Nurse-Led dressings clinics and working autonomously to see, treat and discharge patients, some with complex wounds.
    • Co-ordinating the department when required.
    • Continuously problem solving to ensure safe and high quality patient care.

    Detailed Job Description And Main Responsibilities

    The post holder will take personal responsibility and accountability for ensuring that Trust and local policies and procedures are known and followed and will ensure duties are carried out in accordance with the advisory guidelines and practice set out in the NMC code of Professional conduct.

    The post holder will be responsible for assessment, planning, provision, maintenance and evaluation of patient care.

    The post holder will work within and lead a clinical care team.

    The post holder will autonomously undertake nurse-led clinics (detailed in appendix 1) and assist with various other clinics.

    The post holder will be designated on a regular basis to take charge of the department in the absence of the Supervisory Sister or Sister after two years in post and on an occasional basis within the first two years in post.

    The post holder will take responsibility for the supervision and guidance of other staff including health care assistants and students on a daily basis.

    The post holder will actively contribute to the provision of a good learning environment by taking on the role of mentor/assessor for learners eg. Student nurses and support worker roles.

    Knowledge, Training, Experience And Skills Required

    Registered nurse with current NMC registration to practice.

    • Evidence of continued professional development ie: a professional revalidation portfolio and participation in appraisal and development review.
    • To have achieved a recognised teaching & assessing qualification, or to be willing to work towards
    • Completion of training for additional clinical skills required in the department ie: complex wound dressings/Vacuum Assisted Closure/cannulation & venepuncture
    • Green card training for the use of pumps or machinery relevant to the department.
    • Evidence of attendance and current/valid mandatory training appropriate to the role.
    • Competence in the use of computers and electronic communication systems or expectation to develop existing skills further as the needs of the service dictate.
    • Sound knowledge of health promotion issues to ensure junior staff, patients and carers are provided with education, support and advice as required.
    • Evidence of experience and knowledge of the importance of resource management including protocols for staff utilisation, equipment and products for clinical care.

    Evidence of an ability to critically appraise and utilise research to maintain and improve patient care.

    Main Duties & Responsibilities Of The Post

    Organise and participate in the delivery of high standards of evidence based nursing care.

    • Take regular responsibility to carry out required planned, direct patient care to a high standard without direct supervision in accordance with established nursing procedures and policies.
    • To report to the appropriate members of the multidisciplinary team any significant changes in a patient’s condition or situation or clinic outcome.
    • Responsible for taking charge of the department on a regular basis in the absence of the person who has continuing responsibility.
    • Take personal responsibility for the delivery of care of patients in the department or designated clinic.
    • To contribute towards health promotion for all patients by providing education, support and advice, as necessary Ensure referral to appropriate professionals or agencies as required.
    • Ensure the safe ordering, administration and custody of patients’ medication according to Trust policies and procedures.
    • In the absence of senior staff be responsible for delegation and supervision of appropriate work to other qualified staff, health care assistants and students
    • Where relevant to be responsible for co-ordinating outpatient discharge planning in partnership with the multidisciplinary team eg. Tissue Viability referrals/Community Nurse team/Hospital at Home .
  • Where relevant, responsible for the admission, assessment and preparation of patients undergoing treatment or investigations.
  • Escort patients to other wards or departments as required.
  • To take personal responsibility for ensuring that patients are treated with courtesy at all times, the individual needs of patients are respected, and confidentiality is maintained at all times.
  • Liase effectively with other disciplines or departments as appropriate.
  • To be able to act in emergency situations and to be familiar with emergency procedures including fire and resuscitation. To be responsible for the support and direction of junior staff in such situations according to Trust policy.
  • To be responsible for ensuring provision of a safe and therapeutic environment for all patients, relatives, visitors and colleagues.
  • Ensure the readiness for use of special equipment and the availability of nursing supplies in conjunction with the senior ward nurses.
  • Ensure the safe custody of patients’ valuables and property according to Trust Policies (in emergency situations only).
  • Report to nurse in charge/line manager all complaints, incidents or accidents involving self, staff, patients, visitors and complete relevant documentation according to Trust procedure.
  • Be responsible for reporting and documenting any broken or damaged equipment according to Trust policy.
  • To have detailed knowledge of Manual handling regulations particularly with regards to the moving and handling of patients. To be responsible for ensuring that assessment of patients’ manual handling needs are carried out, and ensure junior staff and students follow these requirements in the course of care delivery.
  • Ensure safe storage and disposal of substances hazardous to health in accordance with Trust policy. Ensure availability of facilities for disposal of sharps, toxic/cytoxic waste in accordance with trust policy.
  • In support of senior staff or supervisory sister, assist with monitoring of stock and report to ensure adequate levels are maintained and that the correct quantities are in supply.
  • Monitor the use of supplies by junior staff to promote efficient use of all supplies.
  • To be aware of the directorate bleep holding function.
  • To have an awareness of the requirements for staffing the ward or department.
  • Undertake aspects of departmental administration as agreed by the Sister or Supervisory Sister.

    Working Conditions / Effort

    Nursing staff may be required to work in any location owned, managed or attributed to the Trust sites depending on the demands of the service.

    • Workloads may be unpredictable on a day-to-day basis and variations in activity should be expected
    • Staff are required to walk or stand for the majority of the shift time.
    • Occasional weekend and evening working may be expected as part of the role.
    • Manual handling of loads and patients using the appropriate aids and assessments will be required in the department.
    • Pushing commodes, trolleys, beds and other furniture according to manual handling policies may be required in the department.
    • Contact with blood and bodily fluids on all shifts.
    • The staff nurse may be expected to deal with distressed, violent or aggressive (verbally or physically) patients and relatives on occasion.
    • The requirement to take responsibility for nursing acutely ill and vulnerable patients at times.
    • Due to the nature of the workload staff will be required to concentrate at a high level often during the shift in relation to carrying out instructions/documentation/communication. This will involve the use of computer screens for periods of time.
  • The staff nurse will be required to be responsible for multiple tasks, delegation, other staff/carers and patients simultaneously frequently throughout a shift.
  • Staff maybe required to act quickly and immediately in the event of emergencies or other unexpected events within the ward area.

    List of Specific Clinical Competencies (Nursing)

    After a period of training and supervision you will be expected to:

    • To be able to manage the department and coordinate the clinics including dealing with staff issues, complaints and compliments, risk assessments, answering queries and delegating appropriately.
  • To assist with procedures such as seroma drainage or biopsies.
  • To carry out Nurse led Dressing Clinic autonomously.
  • Assist practitioners with Biopsy clinics.
  • To carry out complex wound dressings.
  • To carry out Vacuum assisted closure dressing changes
  • To support the management team in implementing any departmental changes.
  • Prepare and support patients prior to, during and following healthcare procedures such as biopsies.
  • Undertake Point of Care testing
  • Green card training to carry out observations and department specific equipment.
  • Obtain, test and record specimens eg; venepuncture/biopsy samples.
  • Chaperone patients in accordance with policy.
  • Confident to work independently and as part of the team.
  • Prepare clinical area prior to, during and following a clinical session.

    Work Experience

    Essential criteria

    • Expectation to undertake training for extended roles required by the ward / department. i.e., Cannulation, venepuncture, ECGs, complex wound dressings, Vacuum Assisted Closure .
    • Currently working in a hospital setting, as a Qualified Nurse or a Qualifying Student

    Desirable criteria

    • Experience working in Plastic Surgery
    • Experience working in an outpatients setting

    Knowledge/Skills/Abilities

    Essential criteria

    • Advanced communication skills, verbal & written, to include report writing and record keeping.
    • Excellent interpersonal skills with patients and the multidisciplinary team.
    • Ability to undertake shift work
    • Ability to work as part of a team
    • Basic computer / IT skills and expectation to develop these in line with the needs of the service i.e. EDMS, Careflow, Email use.
    • Ability to organise own workload
    • Time management/Basic Problem solving skills

    • Sound knowledge of trust policy including manual handling, health and safety and infection control.
