About The Team
The North East Somerset (NES) Social Care Team is seeking a passionate individual to join a team committed to excellent social care. As an Adult Social Care Community Team, we work with a wide range of people with varying needs. This provides excellent opportunities to develop social care skills and knowledge. The team is experienced, supportive, and reflective, and consists of social workers, social care practitioners, and administrators.
About The Role
As an Adult Care Social Care Practitioner, your work will be rewarding. You will undertake assessments and support plans with people who have physical disabilities or other complex needs. This role will involve working with families, carers, and other professionals, agencies, and organisations across the World Heritage site of Bath.

al admission or long-term care. The ideal candidate will have experience working in a health and/or social care setting, knowledge of the Care Act, and the ability to apply it in practice. You will understand what really matters to our service users, families, and their carers, work closely with the community and partner agencies, and support individuals in a positive and responsive way. You will be committed to maintaining your professional standards as our service continues to develop and to enabling the creative use of resources, including assistive technology.
