We have an exciting opportunity for an experienced Social Worker to join our Community Team for People with Learning Disability!

This is a full-time, permanent role with hybrid working, and a salary of £40,192 to £44,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

The Role

You will be working as a Social Worker principally to support young people in transition from Children's to Adult Learning Disability Services.

Your Role Will Involve

  • Located within a multi-disciplinary community team you would be supporting young people with learning disabilities and their families and carers to make a positive transfer to adult services. This would include liaising with Children's services and any other involved agencies.
  • You would be involved in carrying out assessments, undertaking care reviews and supporting care planning.
  • You would take part in a duty rota to deal with incidents, safeguarding alerts and information requests.
  • You would work as a Social Worker to protect and promote the interests of vulnerable people.
  • Undertake safeguarding investigations, Deprivation of Liberty Safeguards assessments, capacity assessments and best interest decisions for adults with a learning disability.

What We Are Looking For

  • A qualified Social Worker with a qualification such as a BA, BSC Diploma or equivalent.
  • Current registration with Social Work England.
  • Experience of working in a health and social care environment.
  • Good understanding of safeguarding protocols.
  • Knowledge of relevant legislation.
  • Committed to working co-operatively as part of a team.
