We are pleased to announce a unique and highly rewarding opportunity for a Specialty doctor to join the Hospital at Home (HAH) and Same Day Emergency Care (SDEC) services at Darent Valley Hospital. This newly created post is part of a pioneering cross-sector expansion initiative designed to enhance our established care pathways. By joining our team, you will contribute to shaping the future of innovative healthcare delivery.
The HAH, SDEC, and Frailty Assessment Unit are strategically integrated within the SDEC Village, facilitating collaboration across multiple specialties. In this role, you will work closely with our emergency department and the medical and surgical directorates to optimize both step-up and step-down care for our patients. As a member of the HAH service, which reports directly to theexecutive team, you will have the opportunity to impact patient outcomes while helping to lead service enhancements.
Engage in daily, consultant-led ward rounds to maintain continuity and quality of patient care. Triage and accept referrals from Same Day Emergency Care (SDEC), EmergencyDepartment (ED), inpatient wards, and General Practitioners, streamlining patienttransitions. Conduct regular reviews of treatments, investigations, and care plans, ensuring comprehensive patient management. Contribute to the development of specialty-specific care pathways, enhancing service delivery. Attend and present at governance meetings to support continuous quality improvement. Perform clinical procedures, including bedside ultrasound, ascitic drains, and lumbar punctures, to expand practical skills.
