Are you a Staff Nurse within an acute care setting looking for an opportunity to work within a Community Care setting whilst utilising your acute care skills to deliver a unique service that aims to bring hospital care to the Patient within their own home? If so, this unique role is for you.
The Acute Care at Home service is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), Royal Wolverhampton Trust (RWT) and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, to provide clinical assessment, monitoring and support, as part of our urgent care pathways.
As a Staff Nurse working within the North Staffordshire team, you will play a key role in supporting hospital admission avoidance and supporting patients to leave hospital sooner than would otherwise be possible. You will deliver high quality, professional evidence-based care when visiting patients across North Staffordshire.
The post holder will work autonomously under the guidance of the Senior Sister/Charge Nurse and be responsible for the clinical assessment of a designated group of patients on the Virtual Ward.
They will carry out face to face visits to deliver care and assess response to treatment plans to inform consultant led programmes of care, always maintaining quality and safety standards.
As part of the role, they will utilise skills such as delivery of IV medications and health monitoring for patients with a wide range of medical conditions across North Staffordshire. They will act as a professional role model to staff and service users contributing to staff education and development plus quality improvement initiates and service development.
You will be required to have access to a vehicle which can be used for work purposes or able to travel as required to meet the needs of the service. Please note not all office locations or client care bases may be reached by public transport?
