Supported Living Support Worker

About The Role

As a Support Worker, you will provide person-centred care to individuals in their own homes, supporting them with a variety of health needs, including:

• Learning disabilities
• Mental health conditions
• Physical disabilities
• Personal care needs
• Complex care requirements
  
  

You will play a vital role in ensuring that those in your care live fulfilling, independent, and happy lives. Your responsibilities will include:

• Assisting with personal care and hygiene routines
• Administering and managing medication in strict compliance with care plans
• Supporting individuals to access the community, engage in social activities, and develop healthy relationships
• Encouraging independence in daily tasks such as meal preparation, household chores, and financial management
• Maintaining a clean, safe, and accessible home environment
• Supporting with day-to-day activities, including attending appointments, shopping, and engaging in hobbies

Assisting individuals in managing emotions and behaviours, promoting positive mental well-being

Providing companionship and emotional support

Supporting individuals in external settings such as hospitals, holidays, or police stations if required

Keeping accurate records and reporting to the Service Manager

Working Hours

We offer flexible shifts to suit your lifestyle, including:

• Days or nights available (We offer outreach support - 3hours - 10 hours per day and full days (12 hours).
• Sleep-in shifts available also.
  
  

Roles available in Bury, Radcliffe, Rochdale, Shaw, Chadderton

This is a variable hour contract. Please include your availability for work in your application.
