We are looking for a forward-thinking individual to join us as Team Manager of our temporary countywide Continuing Health Care Project Team.

This is an exciting opportunity to develop your leadership and partnership working skills. You will be leading a diverse team to carry out collaborative, data-driven work with Health colleagues to complete DSTs or reviews of people to determine eligibility for Continuing Health Care or identify any Joint Funding support. This team aims to improve outcomes in a truly personalised way, and ensure proportionate care and support is in place.

You will be empowering the team to work in a strengths-based, person-centred approach and to innovatively draw on Technology-Enabled Care. This team includes Service Advisors, Community Care Officers, Social Workers and an Occupational Therapist, with support from Service Improvement.

You will need to have relevant experience of managing and supervising staff, particularly in a climate of change and through challenging conversations, as well as experience of working with health partners and in Continuing Health Care. You will also need to have the right values, creativity and ambition, and be able to evidence a commitment to putting people at the heart of everything we do.
