The post holder will provide high-quality, patient-centred acute frailty care within the Humber Virtual Frailty Ward, working closely with the Community Frailty Hub to avoid unnecessary hospital admissions and support safe care in patients usual place of residence. The role involves autonomous and multidisciplinary working to assess, treat, and manage patients with acute frailty-related deterioration, including falls, delirium, mobility decline, deconditioning, medication concerns, and exacerbations of long-term conditions.
Responsibilities include completing frailty assessments and Comprehensive Geriatric Assessments (CGAs), developing timely management plans, making safe clinical decisions in line with NICE and GMC guidance, and coordinating rapid community interventions. The post holder will provide senior clinical input to virtual ward MDT discussions, support admission avoidance and early supported discharge pathways, and assist with proactive escalation and advance care planning, including ReSPECT and Treatment Escalation Plans.
Working across primary care, community services, secondary care, ambulance services, mental health, social care, and voluntary organisations, the role supports a no wrong door approach to integrated frailty care focused on recovery, independence, and preventing unnecessary hospital attendance. The post holder may also contribute to clinical governance, audit, service evaluation, and quality improvement initiatives.
