Main duties of the job
IC24 seeks an experienced Advanced Clinical Practitioner to join our Primary Care team. You will work autonomously, using advanced decision-making skills to deliver safe, effective, and patient-centered care in urgent and primary care settings. Responsibilities include providing advanced clinical care through telephone triage, remote consultations, and face-to-face assessments, diagnosing and managing complex cases, prescribing medications, and coordinating care across services. You will also lead on clinical quality and safety, mentor colleagues, and safeguard vulnerable individuals. Flexible working, including travel to different IC24 sites, is required.

- Professional registration (NMC, HCPC, GPhC) is required, alongside advanced assessment and minor illness qualifications (Level 6 or 7).
