Clinical Nurse Specialist - Community Stroke Team

The Role

The post holder will work autonomously in the community to manage a complex clinical caseload within the Service, applying specialist knowledge and skills relating to their professional role and working as an effective and efficient partner and coordinator in the interdisciplinary team. To work together with other professionals in health, education and social care to provide advice on effective care and health promotion to adults with stroke.

Within the Community Stroke Team the post holder will deliver by reason of their highly developed specialist knowledge, underpinned by theory, practice and experience, a high quality specialist service, providing expert advice, clinical assessment, triage, intervention, case coordination, and education to the support of patients and their carers. The post holder will provide support, guidance and supervision of less experienced professionals e.g. students, Assistant Practitioners (AP) and administrative worker. They will work flexibly according to the needs of the Service

Duration, Location, and Working Pattern:

Be jointly responsible for identifying gaps and pressures within the service e.g. waiting lists, staffing levels, and other resources available and discuss with the Team Lead, implementing remedial action

Knowledge, training, qualifications and/or experience required to do the job:

• Degree/Diploma or equivalent in Adult Registered Nursing
• Registered with the relevant Health Professions Council (e.g., NMC)
• Minimum of 3–5 years post‑graduate experience, including relevant experience working with Adult Stroke
• Excellent interpersonal skills for dealing with patients, families, and co‑workers
• Ability to work independently and as part of a multi‑disciplinary team
• Clean current UK driving licence
• Experience of working within a community‑based multi‑disciplinary team
• Leadership/management experience
• Knowledge of IT systems
