Learning Disability Crisis Response Intensive Support team (LD CRIST):
The LD CRIST are a multi disciplinary team, with learning disability & mental health nurses, health care support workers, psychology, speech and language therapists, occupational therapist and psychiatry.
The LD CRIST provide a service 7 days a week between the hours of 08:00-21:00hrs including bank holidays.
Opportunities have arisen for Band 3 Community Learning Disability Support Worker to join LPT to support people and their families in a community setting.
The primary aim of the team is to prevent unnecessary hospital admissions. LD CRIST will provide rapid assessment, intensive support to people with a learning disability and behaviours that challenge or mental health conditions.
The principle role of the Senior Healthcare Assistant is to assist the healthcare team in the provision and delivery of programmes of patient care under the indirect supervision of registered practitioners. This includes: Taking physical or psychological measurements from named clients/service users as delegated by the team.
