If so, we want you to join in our team! We are currently looking for an Independence advisor to work 37 hours per week across Monday to Friday in our Adult Social Care Connect Team.
About the team:
Adult Social Care Connect is the front door team for Southampton City Council Adult Social Care referrals alongside our corporate Service Centre. Its primary focus is new referrals into Adult Social Care. The team are responsible for completing Care Act assessments and support plans for adults in the community under the Care Act 2014. There is a strong focus on early intervention and prevention and providing clear pathways to our short-term and long-term care services. The team also includes operational safeguarding and completes other statutory duties for example working under section 42 of the Care Act fulfilling statutory duties around safeguarding and carrying out work within the remits of the Mental Capacity Act assessing the capacity of the individuals we work with and supporting decision making within this.

What we offer:
Salary: The salary band for this role is £37,280 - £40,778. The starting salary is £37,280 with annual progression through the salary bands.
