The PCN Manager is a key member of the leadership team, alongside the Clinical Director (approx. one day per week) and board members (appointed by each practice).
The postholder will support strategic planning and ensure that decisions are implemented successfully. This work will reflect the following priorities;
- The development of neighbourhood health, and what this means in reality for the PCN area
- Pilots and longer-term projects supporting the move from hospital to community
- Increasing focus on prevention, and how funding can be secured/released for this
- Digital transformation and innovation

Chapeltown PCN serves a population of approximately 27,000 patients across three member practices;
