The post holder will support residential care home staff to positively manage and meet the health needs of individuals with a Learning Disability within the Croydon Area.
The post holder will work collaboratively to ensure people living in care homes have access to enhanced primary care and to specialist services and maintain their independence as far as possible by reducing, delaying or preventing the need for additional health and social care services
Main duties of the job
To identify and address health inequalities and ensure that long-term health conditions are identified and addressed To assess, advise and contribute to meeting the health needs of people with complex physical health Introduce a Training Analysis of residential care home staff to identify areas of further development and educative support. Promote and support up-take of Annual Health Checks with GP Practices/PCN networks Review quality of Health Action Plans/Health Care Passports - Advise/support/facilitate identified health needsPerson specification
EDUCATION AND QUALIFICATION
Essential
- RNLD or Degree in Registered Nurse Learning Disability
Desirable
- Evidence of post qualification training experience up to MA level in a relevant field
- Evidence of further associated qualifications/CPD in Learning Disability Practice.
KNOWLEDGE AND UNDERSTANDING
Essential
- Knowledge of current research and legislation in the field of Learning Disability Nursing and ability to translate evidence into practice
Desirable
- Knowledge of NHS Long Term Plan Ageing Well Project
Experience
Essential
- 2 years post-registration experience of learning disability nursing
- Experience of nursing in the community/a Community Learning Disability Team
- Experience of working with people with learning disabilities with complex physical health needs.
- Sound knowledge of the principles of clinical governance, national policy and good practice guidance relating to people with learning disabilities. In particular, evidence based practice, clinical audit and the current access to healthcare agenda.
Desirable
- Experience of working with people with a Learning Disability in an acute/hospital setting.
- Experience of working with people with learning disabilities who engage in behaviours that challenge.
- Experience of assessing people with suspected dementia.
- Experience of assessing people with suspected autism.
- Experience of CHC framework and implementation
