We have an opportunity for a Permanent, Full Time Social Worker within our Adult Social Care Connect Team.

Adult Social Care Connect is the front door team for Southampton City Council Adult Social Care referrals alongside our corporate Service Centre. Its primary focus is new referrals into Adult Social Care where no social care provision is currently in place. There is a strong focus on early intervention and prevention and providing clear pathways to our short-term and long-term care services. This service also includes operational safeguarding and supports the delivery of consistent approaches to safeguarding across Adult Social Care.
The social work role and responsibilities include undertaking assessments under the Care Act 2014, Mental Capacity Act 2005 assessments, contributions to multi-agency planning, provide interventions to individuals and their families or carers. Negotiating and coordinating resources and conducting s.42 safeguarding enquires.
