Community Advanced Practitioner – Band 7

Permanent | Full-Time (job share could be considered)

An exciting opportunity has arisen for a Community Advanced Practitioner (CAP) to join the integrated team at Aberdeen City Health and Social Care Partnership. This is a permanent, full-time Band 7 position, ideal for a dynamic and experienced clinician looking to make a meaningful impact in community care.

About The Role

As a Community Advanced Practitioner, you will:

  • Provide direct clinical management of patients within the community nursing caseload.
  • Support community teams with complex palliative care
  • Conduct comprehensive assessments, formulate differential diagnoses, and deliver evidence-based treatment and management plans.
  • Collaborate closely with multidisciplinary teams to ensure high-quality, person-centred care.
  • Support the teaching, supervision, and development of nursing colleagues in advanced practice.

About You

We are looking for an energetic, compassionate, and motivated individual who thrives in a collaborative and evolving environment. You should have:

  • A PgDip or MSc in Advanced Clinical Practice.
  • Essential community experience and a strong foundation in advanced clinical skills across a broad range of settings.
  • A commitment to continuous improvement and service development.
