About The Role
As a Community Advanced Practitioner, you will:
- Provide direct clinical management of patients within the community nursing caseload.
- Support community teams with complex palliative care
- Conduct comprehensive assessments, formulate differential diagnoses, and deliver evidence-based treatment and management plans.
- Collaborate closely with multidisciplinary teams to ensure high-quality, person-centred care.
- Support the teaching, supervision, and development of nursing colleagues in advanced practice.

- A PgDip or MSc in Advanced Clinical Practice.
