We are currently looking for motivated individual to join our friendly team here at Rotherham, this role is 37.5 hours per week on a 6 month contract. The overriding purpose is to support the provision of the highest quality patient care through personal actions and continuous improvement. you will assess, develop, plan, implement and evaluate specialist nursing care programmes related to tissue viability. You will respond to patient assessments, provide timely referrals to other specialists when needed, and participate in MDT discussions. You will act as an advocate for patients and, with patient consent, communicate care plans with family and carers.
To have a central role in the Tissue Viability Team, working closely with medical and nursing colleagues; providing specialist nursing contribution to the clinical management, enabling prompt assessment, diagnosis and treatment of patients presenting acutely and ongoing tissue viability requirements. Ensuring the patients and carers receive holistic, multi-faceted support, co-ordinated care, and all the information they need, throughout the course of their acute or chronic conditions.
