Advanced Clinical Practitioner - Intensive Home Support

An exciting opportunity has arisen for an enthusiastic, motivated and committed Advanced Clinical Practitioner with strong clinical leadership and facilitation skills has arisen to join the Intensive Home Support Service (IHSS).

IHSS is an award-winning multidisciplinary team comprising ACP's, Nurses, Physiotherapists, Occupational Therapists, Nurse Associates/Assistant Practitioners and Medics. The primary aims of the service are to reduce the number of avoidable conveyances to hospital and provide a holistic and comprehensive assessment in the patients' usual place of residence. IHSS is commissioned to provide the 2-hour Urgent Community Response across East Lancashire and Blackburn with Darwen - which falls under the umbrella of Community Virtual Ward. As part of this, both remote patient monitoring for low risk/intervention patients, as well as high acuity/intervention within a Hospital at Home model is provided. IHSS currently also takes a proactive approach in generating referrals from ED via our own deflection team, as well as collaborative working with North West Ambulance Service to replace ambulance attendance for patients deemed suitable for a 2-hour UCR.

Essential

• Post Graduate Qualification in specific speciality or equivalent experience
• Successful application to MSc in Advanced Clinical Practice.
• Recognised teaching or mentorship qualification at degree level or equivalent
• Leadership module or equivalent Leadership study days

• Evidence of clinical experience at a high level in a range of clinical settings.
• Proven experience of working with complex cases
• Up-to-date evidence of continuing engagement in professional development and of reflective practice in a professional portfolio
• Evidence of supervising team members
• Evidence of experience in mentoring/teaching role.
• Evidence of experience working in partnership and collaboration
• Participation in research, audit and evaluation
• Conducting appraisals and agreeing personal development plants
• Leading change and innovation

Knowledge

Essential

• High level of clinical knowledge including current evidence-based practice
• Highly competent to assess patient's requirements to develop case management plans.
• Ability to interpret data from diagnostic tests to adjust interventions appropriately
• Knowledge of chronic disease management
• Sound understanding of the expectations of an advanced practitioner, particularly in terms of service outcomes and their relevant context.

Good understanding of systems in health and social care that are wider than immediate role.

Demonstrate an awareness/understanding of the factors that contribute to good health and the public health priorities of East Lancashire Hospitals Trust

Able to plan proactively, manage and evaluate own learning, to negotiate learning opportunities and establish learning contracts
