Discharge Liaison Practitioner

We are seeking a Locality Discharge Liaison Practitioner to support safe, timely discharge from inpatient mental health services into the South communities of Grantham, Sleaford, Spalding and Stamford. Working across inpatient wards, LMHTs, social care and wider partners, the post holder will coordinate discharge planning, assess ongoing service needs and support smooth transitions to appropriate services. The role plays a key part in reducing delayed discharges, improving patient flow and promoting person centred, recovery focused care across the locality.

Job description

Act as key person for an initial period following discharge from inpatient wards and assess for LMHT suitability longer term.

Transition service users into appropriate services across secondary services or wider VSFCE services as appropriate to a patients needs in a timely manner.

Engage directly with service users to understand their needs and role of LMHT.

Attend ward reviews and liaise directly with ward in person and via MST.

To support the Trusts ambitions to reduce length of inpatient stay, improving flow

Working collaboratively with the multi-disciplinary team, including the bed management team, co-ordinate and facilitate the safe transfer of service users from hospital back into the community.

Communicate and liaise with service users, relatives/carers, health service staff (hospital and community), Social Care and appropriate stakeholders to support the holistic care of an individual.

Provide support, assistance and information to wider team members within a locality.

The post holder will provide professional advice and support to other agencies, stakeholders and members of the multi-disciplinary teams within their locality.

• oRobust understanding of how inpatient services work
• oEvidence negotiation skills in working with other internal and external service providers
• oManaging conflict and negotiation
• oGood understanding of community resources
• oWorking with both inpatient and community social workers and having a broad understanding of social care
• oRelevant experience of working with adults with enduring mental health needs. Or clear evidence of substantial experience of working with individuals in crisis and skills that are transferable to both community/in-patient.
• oEvidence of continuing professional development
