Integrated Neighbourhood Team Coordinator

Integrated Neighbourhood Teams are in the early stages of development across Shropshire Telford and Wrekin. Given Shropshire Community Health Trust's unique position within the footprint, the Trust is keen to take the lead with much of this work. The introduction of this new role is designed to ensure that neighbourhood working is given the priority required to meet the needs of our population.

The Integrated Neighbourhood Team (INT) Coordinator will be a central point of contact and navigator between their designated Primary Care Network (PCN) and the INT multi-disciplinary team (MDT) establishing effective communication processes across primary care, community services, mental health services and the voluntary/third sector.

The INT Coordinator will work in collaboration with the INT core team and will be responsible for liaising with designated leads to manage the coordination of patient-related information to support the MDT's triage and MDT meetings including capturing outcomes and evaluation.

Training & Qualifications

Essential

• Educated to GCSE level or equivalent with grade C or above in English and Maths
• ECDL or equivalent IT qualification / experience
• IT Systems and project management experience acquired through training or experience to diploma equivalent

Experience

Essential

• Experience of administration procedures
• Experience of working in a customer-facing environment, resolving queries and using initiative to ensure effective working relationships across services

Knowledge & Skills

Essential

• Competent in the use of IT software systems, (including Microsoft Office) and handling sensitive personal identifiable data
• Demonstrable evidence of problem solving and resolving issues as they arise in a fast-changing environment
• Able to communicate clearly and sensitively with a wide range of people, including external partner organisations, community teams and patients and their families
• Able to induct new staff into a team, providing training to new colleagues

Other

Essential

• Ability to meet the travel requirements of the post which will include the ability to work across different sites within the Trust
