Exciting new opportunity for a Senior Community Nurse Lead role within Community Service.
We are looking for a compassionate, hardworking and motivated Senior Leader to join our Community nursing team in the South East Essex area on a permanent basis at Band 7. This is an opportunity for a developing leader to gain experience and enhance their skills.
You will have responsibility for managing & leading the community nurses. In addition you will support the team in Triaging calls/referrals and providing unplanned care to patients within the community setting. You will liaise closely with other members of the wider MDT as well as with patients and their families, therefore excellent communication and IT skills are essential.
To provide clinical leadership within the South East community nursing service. To provide specialist advice in community nursing and to be the interface with primary and acute care. To hold close working relationships with partners and to engage in local system transformations.
KEY RESPONSIBILITIES
Demonstrate an awareness of national and local policies that affect health and wellbeing of patients with complex medicalconditions. In addition, ensure team members are aware of new policy requirements.
Lead on or contribute to the development of community nursing policies, procedures and guidelines ensuring they are implemented and monitored.
To ensure learning from audits and complaints are cascaded to appropriate staff, which may influence service policies, guidelines and procedures.
To take an active interest in working parties and groups in the Trust to develop and improve on service delivery, protocols and guidelines
To champion and participate in team, PCN, directorate and initiatives as required with a specific focus on the implementation of the out of hospital model of care and development of locality virtual integrated teams.
Ensures the effective and efficient use of resources e.g. staff supplies within their own sphere of responsibility.
Comply with the EPUT standing financial instructions, standing orders, policies and Code of Conduct for NHS Managers.
Support the Head of Community Nursing with regard to service reviews as necessary for the role, including value for money exercises, to ensure efficient use of all resources and ensuring the most appropriate person undertakes tasks/care.
Responsibility for the supervision and co-ordination of staff within the community nursing team on a daily basis.
Conducts appraisals for the community nurses within locality.
To support the learning and development of others including students and newly qualified nurses undergoing preceptorship.
Identify workforce-planning issues and actively participate in the recruitment, selection and retention of clinical staff.
To participate in the development of performance management systems towards monitoring sickness absence, annual leave and bank usage and take appropriate action as necessary
Implements, monitors and maintains agreed standards of care, reporting to Matron/Head of service when standards are not of an acceptable level.
