Service Coordinator
Location: London (On-site)
Salary: £25,541 to £27,200 per annum, pro rata (plus ILW, if residing & working in London)
Hours: 35 per week (Mon-Fri, 9am-5pm)
Contract: Fixed term contract (until 31st March 2027 with possible extension subject to funding)

As a
Service Coordinator, you'll play a key role in helping people make a safe and successful transition from hospital to home. Leading a dedicated team of staff and volunteers, you'll ensure vulnerable people receive the practical and emotional support they need to regain confidence, maintain their independence, and avoid unnecessary hospital readmission. Working closely with hospitals, health and social care professionals, and community partners, you'll coordinate a service that delivers real impact when people need it most.
This is a highly visible, community-based role involving regular travel across across hospitals and partner organisations within the scheme's coverage area across London to meet service needs. You will also be expected to provide direct frontline support to service users when required, ensuring continuity of service during periods of staff absence or increased demand. - Acting as the main point of contact for hospital and community referrals.

Please note: This is an active, community-based role involving significant travel across London. The successful candidate must be comfortable travelling regularly between locations and undertaking occasional practical support duties to ensure continuity of service delivery.
