Package Description: Advanced Nurse Practitioner - Primary Care The Role Primary care is where advanced clinical skills make the greatest difference. It's where you assess, diagnose and treat patients at the point of first contact. It's where clinical expertise, autonomy and decision-making come together to deliver safe, effective care for people who may otherwise struggle to access healthcare services. At IC24, we're looking for an Advanced Nurse Practitioner who wants to use their skills in a role that combines clinical leadership, autonomous practice and the opportunity to improve outcomes for some of the most vulnerable patients in our community. Based within our Hastings Primary Care Hub, you'll lead on delivering high-quality care to unregistered patients, working as part of a multidisciplinary team to assess, diagnose, treat and navigate patients to the most appropriate care pathway. The role also involves working closely with local partners, including the Seaview Project, supporting individuals experiencing homelessness, those living in temporary accommodation and patients with complex health and social care needs. The position is based in Hastings at the Hastings Primary Care Hub, where you'll provide care for unregistered patients. This includes working within a multidisciplinary team to determine appropriate care pathways and build relationships with local partners to support individuals facing barriers to healthcare access, including mental ill health, substance misuse, and homelessness.

Essential

• First Level Registered Nurse with current NMC registration Independent Prescriber qualification Completed a Physical Assessment of Adults module and a recognised Minor Illness qualification At least two years' experience in General Practice or Urgent Care Excellent clinical assessment and decision-making skills Confidence in autonomous practice and managing clinical risk Experience in service improvement, audit, and clinical governance Effective communicator with patients and colleagues Commitment to continuous professional development and evidence-based practice
