About the role
An exciting new opportunity has arisen within Central Camden Primary Care Network (PCN) for a Care Coordinator to manage patient caseloads and provide coordination and navigation of care and support for patients across health and care services.
Central Camden PCN are a group of 8 member practices who work together at scale and collaborate with others in the local health and social care system. The PCN is highly innovative and delivers services with support from the GP Federation, Camden Health Evolution (CHE) Ltd, that aim to improve the quality of care and outcomes for their patients.
This is a full-time role working 37.5 hours per week, as outlined below. The role will be based across the CHE office, PCN practices and local community venues, with some flexibility for hybrid working depending on service requirements.A 1-hour unpaid lunch break applies on Tuesday, Wednesday, Thursday and Friday.

The following are the core responsibilities of the PCN Care Coordinator. There may, on occasion, be a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

Qualifications
Essential
- GCSE Maths and English
- Educated to A level / NVQ 3 level in a relevant subject or equivalent level of qualification or significant equivalent previous proven experience
- Evidence of continued professional development
Desirable
- Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
- ECDL qualification
- Additional language skills (e.g. Bengali)
Experience

- Experience in creating and maintaining Excel spreadsheets, and databases
- Experience of administrative duties
- Experience in auditing
- Experience in dealing with sensitive and confidential information
- Knowledge/familiarity with medical terminology

- Experience of interacting with patients, clients or customers, both face-to-face and over the telephone
