Care Co-ordinator The Role Great care isn't just about clinical treatment it's about making sure people receive the right support, at the right time, from the right services. As a Care Co-ordinator at IC24, you'll play a key role in helping patients navigate their healthcare journey, working closely with GPs, clinicians and wider community services to ensure care is joined up, personalised and centred around what matters most to each individual. We're looking for a compassionate, organised and proactive Care Co-ordinator to join our Primary Care team at Herstmonceux Integrative Health Centre. This role involves managing a caseload of patients, developing personalised care plans, and acting as a central point of contact to ensure a seamless healthcare experience. You'll work collaboratively with multidisciplinary teams to improve patient outcomes and support continuous improvement in primary care services. We're looking for a compassionate, organised, and proactive individual with experience in health or social care, excellent communication skills, and a commitment to delivering outstanding patient-centered care.

Essential

• Experience working in health, social care or another patient-facing support role. Excellent communication and interpersonal skills. Highly organised and able to manage a varied workload while prioritising effectively. Experience working collaboratively within multidisciplinary teams. Confidence in using Microsoft Office and learning other software. Compassion, proactivity, and a commitment to delivering outstanding patient-centered care. Passion for improving people's experiences of healthcare.
