Role summary
As a Family Front Door Practitioner, you'll play a vital role in helping children, young people and families access the right support at the right time. Working closely with professionals, partner agencies and families, you'll assess referrals, gather and analyse information, and make informed recommendations that help achieve the best possible outcomes.
You'll be a key link between Early Help and Children's Social Care, providing guidance, training and support to ensure referrals are effective and consistent. By promoting collaborative working, supporting step-up and step-down processes, and helping to develop high-quality Early Help practice across Somerset, you'll contribute to continuous improvement and make a real difference to the lives of children and families.
