SOC 6136 (75% match) is an eligible occupation, with a going rate of £33,400, and Torbay and South Devon NHS Foundation Trust is a licensed sponsor. That is our reading of the rules. Whether they issue a Certificate of Sponsorship is still their call.
Worth checking: this could also be SOC 3229 (Welfare and housing associate professionals not elsewhere classified., Social care assessor/health coordinator fits assessment and Care Act work), which is not on the eligible-occupation list. Ask Torbay and South Devon NHS Foundation Trust what the duties actually are.
The Torbay Adult Health and Social Care Team is recruiting Health and Social Care Co-ordinators to work Monday to Friday between 8am and 6pm, with some weekend and bank holiday working. Hours and working patterns can be discussed at interview.
The role involves acting as a single point of contact for community health and social care referrals, undertaking assessments, applying Care Act eligibility criteria, identifying risks, providing advice and signposting, developing support plans, and acting as a Keyworker.
Applicants should have excellent communication and IT skills, a good understanding of the Care Act and eligibility criteria, and experience working within a health and social care setting. The ability to prioritise workloads and produce clear, accurate records and reports is essential.
Please note:
We welcome applications from candidates who do not yet hold a relevant Health and Social Care qualification. Successful applicants may be appointed to a Band 4 developmental position, with full support to achieve the required qualification and competencies. Once these requirements have been successfully completed, the post holder will progress to a Band 5 role.
The HSCC Torquay Team respond in a timely and efficient way to the needs of our Community, by providing easy access to advice/information & support relating to Health & Social Care, including O.T., Physio, Community Nursing and Social Care enquiries and referrals for the Population of Torquay.
The team aim to deal with what they can today and only pass on work where necessary. They respond to incoming enquiries in relation to Community Health and Social Care needs, carry out social care assessments as well as identifying risk, priority and needs. The team co-ordinate hospital discharges for patients and the Intermediate Care caseload. The team work on a rota basis 7 days a week.

Not necessarily. Torbay and South Devon NHS Foundation Trust holds a sponsor licence, but sponsorship for a specific hire is always the employer's decision. Be upfront in your application that you'll require it.
The offer must clear both the general threshold and the SOC 6136 going rate of £33,400. New-entrant and shortage discounts can lower it.
