About the role
We are looking for an experienced dietitian to work within a Primary Care Network in West Trafford. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices.
As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community.
Main duties of the job
You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team. Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, frailty and paediatrics - helping them make lasting changes through person-centred care and behaviour change approaches.
You will also have opportunities to contribute to service improvement, clinical audit, and pathway development. Group education sessions may form part of the role depending on service needs.

Qualifications
Essential
- Fully HCPC and BDA registered practicing Dietitian
- Minimum 3 years of clinical experience in dietetics
- An up-to-date understanding of Health Education Englands Roadmap to Practice
Desirable
- Completion of some level 7 MSc learning
- Previous clinical experience of first contact triage clinical work; even if in the private sector or freelance
- Previous experience of using EMIS / SystmOne
Experience
Desirable
- Experience with working with individuals with Type 2 Diabetes
- Experience with working with frail and elderly patients or in care homes
- Experience in weight management
