Key Worker - Home Support Service

Pay: £26,343.00 per year

Location: Working from AUKS Commonweal office, and premises / homes located throughout the borough of Stockport.

Contract: Initial contract 5 years to 31 March 2027, with an added 2-year extension confirmed till 31st March 2029 subject to a three-month probationary period from the commencement date of employment

Hours of Work: Monday - Thursday 09:00 - 17:00 & Friday 09:00 - 16:30

Pension: Entry into the Age UK Stockport Group Personal Pension Scheme

Holidays: Full time allowance of 22 working days and 8 statutory holidays per annum, plus we close for Christmas!

MAIN PURPOSE OF THE POST:

The Home support worker role will work alongside home support workers on the Home support team, providing practical and emotional support to vulnerable people to assist them to build resilience and wellbeing and to remain living safely in their own home; working in all respects with a strength based approach, alongside clients their families and carers. Working at premises and in communities located throughout the borough of Stockport. This role is supported with guidance from the Home support manager to build existing skills and knowledge.

MAIN TASKS:

• Receive referrals from the Home support manager into the project ensuring all relevant information is collected in accordance with procedures.
• To support on the triage role assisting people to be directed in a timely manner and the best and most appropriate way within the Home support or to other services by referring into the Home support manager who will allocate cases out.
• Ensuring all clients are supported through a strength-based approach to identify and address wants and needs including areas of potential risk. Working with guidance and support to work directly with vulnerable people in a range of situation and circumstances, including people who are at risk of self-neglect, and hoarding.
• Being a second person to assist Home support worker on double up visits
• Deliver services to meet best standards of care including, dignity, equality and diversity.
• Encourage people to develop and work on personal actions plans to give them help to achieve them.
• Provide emotional support and assistance and hands on practical support to each. individual to encourage and assist them to achieve optimum independence.
• Ensure the service provides a high-quality service and achieves its delivery targets.
• Working flexibly and positively in partnership with clients, carers, colleagues, and the Ops manager in collaboration with workers from multi-disciplinary agencies to promote health and wellbeing.
• Demonstrate an in-depth knowledge of working with clients who are at risk of self-neglect, and who have a tendency to hoard, showing empathy and understanding of these behaviours whilst providing specialist advice and practical support in a client’s home, liaising with outside agencies for support with cleans and specialist biohazard cleaning to help to reduce clutter in the home.
• Provide emotional support and assistance and hands on practical support to each individual to encourage and assist to achieve optimum independence.

ADMINISTRATION:

• Use of AUKS charity log system to record and keep up to date case notes. Including outcomes / feedback forms whilst working with and afterwards with the client.
• To handle and process monies in accordance with the organisational financial Policies and Procedures.
• To follow appropriate Policies, Procedures and guidelines for the service to ensure best practice.

ACCOUNTABILITY

• Responsibility for overseeing on case load with guidance from the Home support manager..
• Keep up to date records as required.
• To attend individual supervisions meetings and team meetings as required.
• Training with the home support manager to be familiar and enable full compliance with the relevant Safeguarding policy and procedures.

OTHER

• To be aware of all Age UK Stockport activities and work constructively as part of the staff team.
• To work positively with Age UK Stockport services and activities to assist best use of resources and to positively promote Age UK Stockport at all times.
• To carry out such other duties as deemed appropriate and may be reasonably required by the Chief Executive.
