An exciting opportunity has arisen to join our multi-disciplinary PCN Team, we are looking to expand our team by seeking to employ a qualified General Practitioner to work alongside our practices and PCN staff.
Our PCN staff currently includes Clinical Pharmacists, Social Prescribers, Health & Wellbeing Coach, Mental Health Practitioner, Paramedics, First Contact Physiotherapist, GPAs, Pharmacist Technicians, PCN Manager/Digital & Transformation Lead and a Care Home Team.
The successful applicants will work alongside the PCN and practice teams to support healthcare professionals, patients, and carers.

Lostock Hall Medical Centre
Other associated PCN sites (for example Care Homes and Home Visits)

Patient Management
- Create opportunities to address patients care needs in the most expedient way, taking into account that continuity of care and access to services are often in direct conflict.
- Encourage patients to be aware of the service pressures within primary care and to manage their expectations of what we can realistically deliver in a manner that is fair and proportionate for all care priorities.
- Make improved use of the telephone and other technologies. Encourage patients to rely on face-to-face consultation only where there is a clinical need to do so.
- Communicate and reassure patients of the need to offer alternative care options where appropriate and safe to do so.
- To immediately highlight any concerns, you may have in respect of safety or quality of patient care to the Clinical Director and Practice Partners.
- Monitor and evaluate adherence to local chaperoning policies
Confidentiality
- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the Practice/PCN policies and procedures relating to confidentiality and the protection of personal and sensitive data.
- The post-holder should respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role.
- The post-holder will comply with all relevant information governance, confidentiality and data protection requirements, including the UK General Data Protection Regulation (UK GDPR), the Data Protection Act 2018, the Access to Health Records Act 1990 where applicable, and any subsequent or amending legislation, together with relevant NHS information governance policies and standards.
Data Protection and Employee Information As your employer, the Practices/PCN will need to collect, use and retain personal information about you for purposes connected with your employment. This may include information required for payroll and pension purposes, contact and emergency contact details, references, qualifications, training records and records relating to your employment and career with the PCN.
Your personal information will be processed in accordance with the UK General Data Protection Regulation (UK GDPR), the Data Protection Act 2018 and other applicable data protection legislation. Information will only be collected, used, retained and shared where there is an appropriate lawful basis for doing so and in accordance with the Practices'/PCN's relevant data protection and privacy policies.
Managing information
-Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information.
- Review and process data using accurate coding to ensure easy and accurate information retrieval for monitoring and audit processes.
- Understand responsibility of self and others to the practice and PCN regarding the Freedom of Information Act 2000.
- Makes accurate record keeping in EMIS and makes effective use of the clinical system by ensuring the consistent and accurate entering of data and use of SNOMED Codes.
- Facilitates the use of up-to-date information technology to collate accurate and timely information as and when required by the practice.
- Works within the Caldicott and Data Protection principles.
- Assist in the development of practice protocols for the practice and clinical protocols for the clinical team.
- Alert other team members to issues of quality and risk.
Information Security
All staff are responsible for ensuring that personal and confidential information relating to patients, staff and other individuals, whether held electronically or in paper form, is handled securely and confidentially at all times. Staff must comply with the UK General Data Protection Regulation (UK GDPR), the Data Protection Act 2018, other applicable data protection legislation and the Practices'/PCN's information governance, confidentiality and information security policies and procedures. Computer passwords must not be shared either between systems or users. The Practices/PCN may undertake appropriate monitoring of organisational systems, networks, devices and communications for legitimate purposes, including information security, system management and compliance with organisational policies. Any monitoring will be undertaken lawfully, transparently, necessarily and proportionately, in accordance with applicable data protection legislation and the Practices/PCNs relevant policies and privacy information.
Access and usage of practices/PCNs computers must be in accordance with the Practices/PCNs Policies. Approved secure methods /procedures are to be used for all electronic transfer of personal data. This is in order to protect the practices, patients and staff, and its reputation and to ensure that it complies with the laws and other guidelines.
Records Management
As an employee you are legally responsible for all records that you gather, create, or use as part of your work within the Practices/PCN, and they remain the property of the Practices/PCN. This includes patients, financial. personal and administrative records, whether paper based or on computer. you have a legal duty of confidence to all service users and you should consult and ask for guidance from your manager if you have doubt about the correct management of records with which you work. All staff have a responsibility to ensure information quality standards are achieved.
Training/Meetings
- Attending training and events organised by the Practices/PCN or other agencies, where appropriate.
- Participate positively and actively in clinical and Practice/PCN meetings.
- The PCN and Practices have a strong ethos around training and development of others, therefore the successful candidate will be willing and able to support medical students, and other training roles.
Personal/Professional development:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development demonstrating skills and activities to others who are undertaking similar work.
- Develops and maintains up to date personal development plan and professional portfolio for purposes of annual individual performance review, appraisal, and revalidation.
- Is able to develop own knowledge and clinical practice to meet objectives and changes in service.
- Manage own time, workload, and resources effectively.
- To provide education and training to other staff and Registrars as appropriate.
- Operate a high standard of probity.
- A commitment to life-long learning and audit to ensure evidence-based best practice.
- Awareness of and compliance with all relevant practice policies/guidelines, e.g., prescribing, confidentiality, data protection, health, and safety.
- Maintain continued education by attendance at course and study days.

Research and Development
- Is able to identify, critically appraise and present significant events related to own performance and discuss within the clinical team.
