As a Community Care Officer, Your Responsibilities Will Include

• Identifying Goals: Work with clients to discover their personal goals and aspirations.

Coordinating Care: Organise a range of outcomes to meet individual needs.

Promoting Independence: Encourage clients to take control of their own care and support.

Collaborating with Professionals: Work alongside other professionals to provide comprehensive care.

Managing Caseloads: Effectively manage your caseload to ensure timely and quality support.

To Excel In This Role, You Will Need

• A relevant qualification in social care or a related field.
• Experience in adult social care or a similar environment.
• Knowledge of strength-based approaches and person-centred care.
• 2 years of adult care experience
